Membership Agreement
This Agreement (“Agreement”) is entered into by and between Vancouver Pet Urgent Care (“Clinic”) and the undersigned individual (“Member”) for enrollment in the Membership program. This voluntary agreement is not required for services to be provided, and electing to participate in this Membership Program does not influence medical recommendations or treatment received by the Member’s pet. By signing this Agreement, the Member agrees to the following terms and conditions outlined below.
1. Membership Plan
Annual Fee: $300 per year Eligibility: Allows enrollment of two pets (canine/feline) Benefits: Unlimited examinations 10% off urgent surgical procedures (maximum discount of $200 per pet per procedure)) 10% off diagnostics and treatments Priority for triage
Payment
Annual Payment: A one-time payment covering the full 12-month membership term.
3. Membership Terms and Conditions
Non-Refundable: Membership fees may be refundable within the first thirty days, after which the plan will remain intact for the remainder of the terms provided. If services are used, the plan will no longer be refundable. Discount Limits: Certain services, such as major surgeries or specialty consultations, may have a discount cap or no discount. For example, surgical discounts are capped at $200 per procedure cost. Specialty services from outside sources are not covered under any membership plan. Cremation services are not covered under membership plan. Automatic Renewal: This membership will automatically renew for another term at the end of each membership period unless you cancel it at least 30 days prior to the renewal date. You will be notified via email or mail of the upcoming renewal and your right to cancel (RCW 19.158). Cancellations: All cancellation requests must be submitted in writing via the online form found on our website. The member must provide full details of the reason for cancellation. Cancellation requests should be acknowledged by a representative of the clinic within 5 business days. Verbal requests will not be valid unless followed with the online request form. Legal Considerations: The Clinic will collect and store personal information provided by the member solely for the purpose of fulfilling the terms of this Agreement and will not share this information with third parties except as required by law. Dispute Resolution: Any disputes arising under this Agreement will be resolved through binding arbitration in [jurisdiction]. The parties agree to share the costs of arbitration equally.
4. Urgent Care Services Covered
Non-Critical, Urgent Care for Sick Pets: Care for pets experiencing urgent, but non-life-threatening issues. Sedation and Anesthetic Treatments: Sedation or anesthesia for stable patients who need procedures or treatments that require being sedated or anesthetized. Diagnostics: Tests and diagnostic services, such as lab work, imaging, or other necessary exams to identify the pet's condition. Humane Euthanasia: Euthanasia services for pets that are suffering and require end-of-life care in a compassionate manner. Stabilization for Critically Ill Pets: Initial stabilization for critically ill pets to keep them safe until they can be transferred to an emergency facility capable of providing the necessary critical care.
5. Services Not Covered or Provided
Preventive Care: Preventive treatments, such as vaccinations, routine check-ups, or wellness exams, are not provided under this urgent care membership. Critical Care: The facility cannot provide care for life-threatening situations that require intensive resources like oxygen therapy, blood products, or 24-hour care. Daytime Monitoring and Care: Ongoing monitoring and treatment during regular daytime hours should be managed by your primary care veterinarian or another facility that offers full daytime care services. Transfer to Emergency Facility: In critical cases, if your pet requires more intensive care, the facility will help refer your pet to an emergency facility that can provide the necessary support. You will need to transport your pet for continued care. Cremation Services: The membership plan does not cover general or private cremation services. This Clinic specializes in providing immediate care for stable pets. The Clinic is not intended nor equipped to provide critical care for life-threatening conditions or routine wellness care.
6. Death of Pet
In the event of the death of a pet covered under the Membership Program, the following terms apply: Notification of Pet Death: Member Responsibility: The member must inform the Clinic promptly of the pet's death. The notification should be provided in writing or through the Clinic's designated communication channels. Effective Date of Coverage: Coverage for the deceased pet will end on the date the Clinic is notified of the pet's death. Any benefits, including discounts or priority scheduling, will no longer apply to that pet. Replacement of Pet: Adding a New Pet to Membership: If a member's pet passes away and they wish to replace the pet with a new one, the member can add a new pet to the membership at no additional cost for up to 30 days following the death. This is applicable only to members who have paid for the full year. After 30 days, the addition of a new pet will require an upgrade to the appropriate plan or an adjustment to the membership fee. Pro-Rated Refund (Under Special Circumstances): Refund for Death of Pet: If a pet under the membership passes away and the member wishes to cancel their membership early (prior to the membership year’s end), a pro-rated refund may be provided. The refund will be based on the unused portion of the membership fee, minus a $50 administrative fee. This option is only available if the membership includes a single pet, and the member has notified the Clinic within 30 days of the pet's death. Eligibility for Refund: All requests for pro-rated refunds must be documented and submitted in writing by the member. Verbal requests will not be accepted. Written requests can be submitted via email (info@vancouverpeturgentcare.com) or online cancellation form at the Clinic. Refunds for cancellation due to the death of a pet are only available if the notification is made within 30 days of the pet's death. If more than 30 days have passed since the pet's death, no refund will be issued. Condition of Services Rendered: Pro-rated refunds will not be offered if services are rendered after the cancellation request was received. Refund Calculation and Notification: Upon receiving a valid cancellation request, the Clinic will calculate the refund based on the unused portion of the membership fee, minus a $50 administrative fee. The member will be notified of the refund amount within 10 business days. Refunds will be issued to the original payment method unless otherwise requested in writing by the member.
7. Financial Coverage and Limitations
Financial Coverage Limits: Discount Application: All discounts on services are subject to the conditions outlined in the membership agreement. However, certain services may be subject to a maximum cap. For example: Surgical Discount: A discount of 10% is applied to surgical procedures with a cap of $200 for the total cost of single surgical procedure. Emergency or Unforeseen Expenses: Financial Responsibility: While memberships offer discounted services, they do not cover the full financial responsibility for any procedures or treatments. Members are responsible for covering the full costs of services beyond those discounted under the membership terms at time service is rendered. If transfer to an emergency facility is warranted for the health of your pet, these membership plans will not cover the costs of care at those alternative facilities. Additionally, any third-party or specialty services will incur costs that are not covered under the membership plans.
8. Acknowledgment and Signature
By signing this Agreement, the Member acknowledges that they have read, understood, and agreed to the terms and conditions of the Membership Program, including the payment options, benefits, and membership terms.
